Before you register, check you live in our catchment area. Our practice is at 276 Dudley Road, Birmingham B18 4HL.
To check if your address is in our catchment area, contact us or phone 0121 456 3322 and tell us your postcode.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery please fill out the online registration form below.
Please complete this in full only leaving sections blank that do not apply to yourself.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We aim to process these forms within 5 working days. However during busy periods this may take up to 10 days. Please contact us if you have not heard anything after 2 weeks.
If you do not have access to the internet, or have difficulty in completing the form online, please attend the practice and our reception team will be able to assist in registering.
We encourage you during the registration process to sign up for our online services or register with the NHS App to allow you to book appointments, request medications and view results at your convenience.
